Lake County Commissioners will be offering our annual Health Screening and Personal Health Survey because your health matters.

The Board of Commissioners will be offering a wellness discount for the employees who participate in the annual health screening on the employee’s portion for the medical, prescription and dental premiums.

If your spouse is covered on the County’s health insurance and plans on attending the Health Fair, he or she must register and reserve an appointment as well.

New this year, you can log back into your account and view your screening results. You may then request to have your results submitted to your primary care physician by giving the physicians name and office fax number. The results will then be auto faxed to the physician.

Remember the health screens are by APPOINTMENT ONLY. No walk-ins will be accepted. However, if you would like to receive a flu shot only, no appointment is necessary.

Remember what email address you used for your registration for your appointment. That is the email address where your appointment confirmation and reminders will be sent.

Save your confirmation email so if you need to change your appointment for any reason, you can use the confirmation email to re-log back into your appointment to change it.
